Terms and Conditions
By creating an account at www.ppsrxportal.com and selecting “Opt in to receive Text Message Notifications from Premier Pharmacy” under “Your Account”, or by providing PPS with your mobile phone number, you will receive text messages from Premier Pharmacy.
You must provide a telephone number with an area code within the 50 United States or the district of Columbia. By doing so you are enrolling in the Good Health Inc., Premier Pharmacy Services (“PPS”) Prescription Messaging program and agreeing to the following Terms and Conditions: Upon enrollment, you certify that you are the patient (18 years or older) and consent to enroll or have patient’s consent to enroll. The PPS Prescription Text Messaging Program is a subscription service and is effective immediately upon enrollment. By joining this subscription text message service, you are aware and agree that these text alerts will be sent to the mobile number you have provided to PPS during the enrollment process. PPS does not impose a separate charge for the text messaging program services, but your service provider’s message and data rates may apply. You are solely responsible for all message and data charges that you incur, please check with your provider for detailed information regarding such charges. Text alerts will be sent via automated technology varied frequencies however, you will typically receive text alerts when there is updated information about your prescription(s). When you agree to enroll, you will receive these automated messages that may include: alerts when you have been approved for financial assistance, when your prescription is ready for delivery and when your prescription has been sent out for delivery or a reminder when a refill is due for your current prescription(s). Participation in the PPS Prescription Messaging Program is optional and does not require a purchase. A link to an external webpage (www.ppsRXPortal.com) will be provided within the text alerts and will require an initial account setup or login with preexisting credentials. The account set up will require you to enter the mobile number associated with your patient account at PPS, set up of a unique password and input of additional identifying information to gain access. Once registered, you will have access to detailed information regarding your prescription(s) via the designated webpage. By joining PPS Prescription Text Messaging Program, you are aware and agree that all text alerts you receive from PPS may include limited personal information about your prescriptions. You are aware that if someone else has access to your mobile phone or access to your carrier account they will be able to view information regarding your prescriptions. Access to the external webpage, will require entering the username and password you have previously set up. By joining you are aware and understand that because the messages are sent without encryption, there may be a security risk. Participating carriers include AT&T, T-Mobile®, Verizon Wireless, Sprint, Boost, U.S. Cellular®, MetroPCS, Virgin Mobile USA, Unicel, Centennial, nTelos and Cricket. T-Mobile® is not liable for messages that are delayed or messages that are undelivered. PPS Prescription Messaging program may not be compatible with all cell phone models. You may opt out of PPS Prescription Messaging text alert program at any time by replying STOP to any text message, by unsubscribing once logged into the portal, or by calling 800-540-4700, providing written communication to Premier Pharmacy Services at 410 Cloverleaf Drive, Baldwin Park, CA 91706, or contacting us at info@premierpharmacy.com Once you submit a request to opt out, you will receive a final text alert from PPS confirming your decision to opt out. If you want to re-enroll at any point, you may login to your portal and under “Your Account” click “Opt in to receive Text Message Notifications from Premier Pharmacy” upon which additional text alerts will be sent to you. PPS may change any portion of its text messaging program without notice or liability
